What We Need You To Do:
- In Order for Miracle Kidz to benefit from the My School program we need to sign up as many people as possible to the MySchool MyVillage MyPlanet Program.
- Simply Click on the Link Below & Download and Complete the Application Form:
MySchool SUPPORTER CARD APPLICATION FORM - When completing the form please be sure to List Miracle Kidz Safe House as your Nominated Beneficiary
- Once complete please scan and email your Supporter Card Application Form To Miracle Kidz Safe House or hand the completed form in at your neaest Woolworths branch
- Note if you already have a MySchool Card you may select Miracle Kidz Safe House as a beneficiary online by simply logging into your MySchool Card Account and selecting Miracle Kidz Safe House from the beneficiary list.
